What information do I collect?

I collect information from you when you request an appointment/treatment plan, make a purchase, fill out a form or respond to a survey.

Any of the information I collect from you will routinely be used for treatment, payment, quality monitoring, important forms and purchases.

I implement a variety of security measures to maintain the safety of your personal information when you set an appointment, enter, submit or access your personal information.


I offer the use of a secure server. All supplied sensitive/credit information is transmitted via Secure Socket Layer (SSL) technology and then encrypted into our payment gateway providers database only to be accessible by those authorized with special access rights to such systems, and are required to keep the information confidential.

However, non-personally identifiable visitor information may be provided to other parties for marketing, advertising or other uses.

Google’s advertising requirements can be summed up by Google’s Advertising Principles. They are put in place to provide a positive experience for users.

We use Google AdSense Advertising on our website.

Google, as a third party vendor, uses cookies to serve ads on our site. I am in compliance with the requirements of COPPA (Childrens Online Privacy Protection Act), we do not collect any information from anyone under 18 years of age. My website/page services are all directed to people who are at least 18 years old or older.
